Covid-19 Client Consent:
Massage/bodywork involves maintained touch and close physical proximity over an extended period of time. While we follow strict hygiene practices recommended by the CDC, there may be an elevated risk of disease transmission, including COVID-19. Please notify us of any new symptoms or direct contact with anyone displaying symptoms of COVID-19. We commit to communicating our overall health and ask that clients do the same to decrease the spread of COVID-19. As a matter of public health and safety, masks are required in the treatment room at all times. This is to help protect people with weak immune systems who are at high risk of contracting illnesses. We unfortunately cannot offer treatment to those who are unable to meet these guidelines. Thank you for respecting these measures, and for doing your part to keep our community safe.
Sickness:
Massage & bodywork is not appropriate care for infectious or contagious illness. Please cancel your appointment as soon as you are aware of an infectious or contagious condition. (e.g. cold, flu, lice, etc.) We ask clients to wait until after illness before rescheduling all appointments. The sooner you are feeling better, the sooner you can be rescheduled. Thank you for your cooperation.
Tardiness:
Appointment times are as scheduled and cannot extend beyond the stated time to accommodate late arrivals. Contact us as soon as possible if you are running late to your appointment. Please allow for extra time before your first session to fill out the necessary paperwork. Thank you for your cooperation.
Cancellation:
48 hour notice is required for ALL canceled appointments to ensure availability to other clients. If you are unable to give 48 hour notice due to illness, please notify us as soon as possible. Because mistakes happen, we will do our best to fit you in the same day as your scheduled appointment. Repeat offenders may be 1)charged a “Cancellation Fee” of 25% of the scheduled session, or 2)asked to prepay up to 50% of their session.
Missed Appointments/No Shows:
We understand that unexpected illness or injury may cause missed appointments and ask all clients to contact the office as soon as possible. A client is considered a “No Show” 24 hours after a scheduled appointment without an attempt to contact the office. “No Show” clients will be sent an invoice for 25% of their missed session, and asked to prepay 100% of all future appointments to continue receiving services.
Event Massage Terms:
We are so excited to be a part of your next event. We will finalize all event details before sending out contracts and invoices. Clients will have 48 hours to confirm and sign contracts. A deposit of 25% of the total invoice is required for all reservations, and will go toward the total invoice amount. Payment in full is due at the event unless other terms are agreed upon. Any cancellations within 10 business days of a confirmed event will forfeit their deposit as payment.
Gift Card Terms & Conditions:
Gift Cards and E-Gift Cards don’t expire. They are valid for goods and services with Healing Lotus Massage. They may not be exchanged or redeemed for cash. Lost or stolen gift cards cannot be replaced. For questions please contact us directly.
